We're seeking a compassionate and skilled Physical Therapist to join our team at Providence Medical Foundation. As a Physical Therapist with our team, you'll work closely with our patients' Primary Care Providers and specialists to provide the highest level of care and ensure a smooth transition to the care they need.

We've been serving our communities for over 8 years, and we welcome patients of all ages and cultures. Our staff is diverse and speaks multiple languages, including Spanish and Hindi. We specialize in orthopedic, neurological, and cardiac conditions, as well as pre and post-surgical orthopedic conditions.

Required qualifications:
- Bachelor's Degree
- Graduate of an accredited Physical Therapy professional program.
- California Physical Therapist License upon hire.
- National Provider BLS- American Heart Association upon hire.
- New Grad or entry level experience within an ambulatory care facility or medical group practice.
- Experience with program planning, implementation, evaluation and education training.
Preferred qualifications:
- Master's Degree or Ph.D.
- 1 year of Experience in outpatient Physical Therapy setting.
- Experience with CARF and Title XXII standards.
- EMR experience.
- 1 year of Experience in outpatient Physical Therapy setting.
- With California Physical Therapist License
- Experience with program planning, implementation, evaluation and education training.
- Graduate of an accredited Physical Therapy professional program.
