Physical Therapist (PT) - Home Health

What You'll Do as a Physical Therapist

  • Deliver high‑quality, in‑home physical therapy services
  • Evaluate patient functional needs and develop individualized care plans
  • Collaborate with interdisciplinary care teams to achieve optimal outcomes
  • Adjust treatment plans based on patient progress
  • Supervise and support therapy assistants and staff
  • Maintain accurate, timely clinical documentation
  • Serve as a trusted clinical resource within the community

This role is physically active and hands‑on, involving lifting, transferring, and supporting patients in a home setting.

What We're Looking For

Graduate of an APTA‑approved Physical Therapy program

Current, unrestricted PT license in the state of practice

Minimum 1 year of experience in a healthcare setting

Willingness to travel within the assigned coverage area (up to 50%)

Valid driver's license, reliable vehicle & auto insurance

Ability to lift, push, pull, and support up to 50 pounds

Position reports to: Branch Director / Clinical Leadership Team
