💙 What You’ll Do as a Physical Therapist

Deliver high‑quality, in‑home physical therapy services
Evaluate patient functional needs and develop individualized care plans
Collaborate with interdisciplinary care teams to achieve optimal outcomes
Adjust treatment plans based on patient progress
Supervise and support therapy assistants and staff
Maintain accurate, timely clinical documentation
Serve as a trusted clinical resource within the community

This role is physically active and hands‑on, involving lifting, transferring, and supporting patients in a home setting.

✅ What We’re Looking For

✔ Graduate of an APTA‑approved Physical Therapy program✔ Current, unrestricted PT license in the state of practice✔ Minimum 1 year of experience in a healthcare setting✔ Willingness to travel within the assigned coverage area (up to 50%)✔ Valid driver’s license, reliable vehicle & auto insurance✔ Ability to lift, push, pull, and support up to 50 pounds
