We’re Hiring!We’re looking for a motivated and compassionate Physical Therapist (PT) to join a Skilled Nursing Facility in Sonoma, CA. This 13-week travel assignment offers competitive weekly pay, flexible scheduling, and the opportunity to make a meaningful impact in patient care. Whether you’re an experienced SNF therapist or a new graduate with skilled nursing clinical rotations, we encourage you to apply.

What You’ll Do:

·        Evaluate patients and develop individualized plans of care using strong clinical reasoning

·        Deliver age-appropriate therapeutic interventions

·        Modify treatment plans based on patient progress and clinical findings

·        Document evaluations, treatments, and patient progress according to facility guidelines

·        Discharge patients when goals are met or services are no longer medically necessary

·        Collaborate with interdisciplinary team members to ensure high-quality patient outcomes

What You’ll Need:

·        Active California Physical Therapist license (or eligibility)

·        Skilled Nursing experience preferred

·        New graduates with SNF clinical rotations welcome

·        Current BLS certification from the American Heart Association (or ability to obtain upon offer)
