Local Travel Physical Therapist Assistant (PTA) Skilled Nursing Facility (SNF) 13-Week Contract | South of San Jose, CaliforniaWeekly Compensation: Up to $2,100 Based on ExperienceConnected Health Care, a premier recruitment and staffing firm, is partnering with a respected Skilled Nursing Facility (SNF) in San Jose, California, to recruit a dedicated and compassionate Physical Therapist Assistant (PTA) for a 13-week travel assignment. This is an excellent opportunity for therapy professionals seeking competitive pay, valuable clinical experience, and the chance to work in a supportive rehabilitation environment.Job Responsibilities
- Implement physical therapy treatment plans developed by the Physical Therapist*
- Assist patients in improving mobility, strength, balance, endurance, and functional independence*
- Provide therapeutic exercises, gait training, transfer training, and mobility interventions*
- Monitor patient progress and accurately document treatment outcomes*
- Educate patients and families on exercise programs, mobility techniques, and injury prevention*
- Collaborate with physicians, physical therapists, nursing staff, and other healthcare professionals*
- Maintain compliance with facility policies, state regulations, and professional practice standards*
- Support patients in achieving maximum functional mobility and independence*
- Maintain accurate and timely documentation within the electronic medical record (EMR)
- Associate Degree from an accredited Physical Therapist Assistant (PTA) program*
- Current California Physical Therapist Assistant (PTA) license required*
- Current BLS (Basic Life Support) certification required*
- Previous Skilled Nursing Facility (SNF) experience preferred*
- Strong communication, documentation, and patient care skills*
- Ability to work collaboratively within an interdisciplinary rehabilitation team
