LOCATION: FORT MOHAVE, AZ
| CLIENT: | Valley View Medical Center 5330 S Highway 95 Fort Mohave Arizona 86426 |
| SETTING: | PTA - OP My be asked to float to MedSurg or IP Rehab as needed |
| START DATE: | ASAP |
| GUARANTEE: | 36 hrs/wk |
| ASSIGNMENT LENGTH: | 13 weeks |
| REQUESTED EXPERIENCE: | Min 1 yr of experience; 2+ preferred |
| STAFF: | Will discuss on PIV |
| EMR: | ReDoc & MedHost |
| CASELOAD / PATIENT SCHEDULING: | Will discuss on PIV |
| SCHEDULE: | 8-5 with rotating weekends, we can work to flex off a day when having to cover weekends |
| ADDITIONAL INFO: | Past travel client Dress code: Teal scrubs Parking Location: Back row in front of building where PT is located |
