Location: Petersburg, WVSalary: TBDDescription:

Key Responsibilities

Provide physical therapy treatments to patients under the supervision of a licensed Physical Therapist

Follow and implement individualized Plans of Care

Deliver care in a home health setting

Accurately document patient visits and progress (experience with Homecare Homebase (HCHB) preferred)

Communicate effectively with supervising PTs and interdisciplinary team members

Maintain professionalism, compassion, and patient‑centered care in all interactions

Required Qualifications

Active Physical Therapist Assistant (PTA) License – West Virginia

Previous experience as a PTA

Ability to work independently in the field while following clinical direction

Preferred Experience

Home Health experience

Proficiency with HCHB documentation system
