Physician Assistant (PA) or Nurse Practitioner (NP)
Hybrid Role – In Office + Remote
Alliance Medical ClinicService Area: North Phoenix (Anthem) – West Valley – Scottsdale
Alliance Medical Clinic and Ternion Physician Group are seeking a compassionate and motivated Physician Assistant (PA) or Nurse Practitioner (NP) to join our growing integrated care team.
This is a hybrid clinical role combining in-office care with remote and on-site visits to assisted living communities, focused on the management of dementia, chronic illness, and palliative care needs in geriatric populations.
Position Overview:
The PA or NP will provide comprehensive medical care to patients in:
- Outpatient office settings.
- Assisted Living Facilities.
- Memory Care Communities.
- Group Homes.
- Home-based or Telehealth visits (as appropriate).
This role includes a mix of in-person visits and remote care coordination, supporting medically complex and cognitively impaired patients with a strong emphasis on:
- Dementia evaluation and management.
- Chronic disease management.
- Goals of care discussions.
- Palliative-focused treatment planning.
- Medication reconciliation.
- Care coordination with families and facility staff.
Providers will participate in interdisciplinary team meetings, quality initiatives, and care conferences with facilities and outpatient teams.
Schedule:
- 4–5 days per week.
- Combination of:
- In-office clinic days.
- Assisted living / memory care visits.
- Remote documentation or telehealth visits.
Primary service area includes:
- North Phoenix.
- Anthem.
- West Valley.
- Scottsdale.
Required Licensure / Certifications:
- Active Arizona License as:
- Physician Assistant or.
- Nurse Practitioner.
- Board Certification (Family, Internal Medicine, Adult-Gerontology preferred).
- Current BLS Certification.
- DEA License.
Personal Qualifications:
- Compassionate and patient-focused.
- Comfortable managing geriatric and dementia populations.
- Strong communication skills with families and caregivers.
- Self-motivated with ability to work both independently and within a team.
- Interest in palliative or supportive care models.
