Job DetailsWHAT YOU WILL BE DOING* • Provide comprehensive primary and ambulatory care to a diverse patient population * • Obtain medical histories, perform assessments, diagnose conditions, and interpret clinical findings * • Develop and manage evidence-based treatment plans in accordance with established clinical standards * • Educate patients and collaborate with physicians, nurses, behavioral health professionals, and other members of the care team * • Maintain timely, accurate, and thorough clinical documentation within the electronic health record * • Support organizational quality measures and continuity-of-care initiatives SKILLS YOU NEED* • Required: Active Connecticut APRN or Physician Assistant license * • Required: Graduate of an accredited Nurse Practitioner or Physician Assistant program * • Required: DEA and Connecticut controlled-substance registration, or eligibility to obtain both * • Preferred: Practice experience as a providerInterested in hearing more?
