Job Description

Make a Lasting Impact Where History and Healthcare Meet

Join Fortis Care in partnership with Sadie G. Mays Health & Rehabilitation Center—one of Atlanta's most historic and deeply cherished skilled nursing institutions. For over 70 years, Sadie G. Mays has served the community with compassion, dignity, and excellence. We are looking for a mission-driven Nurse Practitioner (NP) or Physician Assistant (PA) who is passionate about geriatric and post-acute care and dedicated to improving quality of life for vulnerable patients.

In this role, you will serve as the primary clinical provider dedicated to a single facility, giving you the stability to build meaningful, long-term relationships with patients, families, and staff. This position is explicitly structured for work-life balance and clinical focus:

  • Monday–Friday, Day Shifts Only
  • Zero Night or Weekend Call
  • Single-Site Dedicated Focus (No driving between multiple facilities)

You will manage both acute and chronic conditions, guide post-hospital transitions, and partner closely with facility leadership to deliver compassionate, high-touch clinical care.

Key Responsibilities

  • Provide compassionate, evidence-based medical care to patients at Sadie G. Mays, including assessment, diagnosis, treatment, and ongoing management.
  • Direct complex chronic disease management and sub-acute rehab care, reducing unnecessary hospital readmissions.
  • Lead interdisciplinary care planning alongside facility nurses, therapists, social workers, and families to support patient-centered goals.
  • Support smooth transitions of care through post-admission evaluations, comprehensive medication reconciliations, and family conferences.
  • Complete timely, accurate clinical documentation in the facility EHR.
  • Serve as a clinical champion for quality improvement initiatives that directly enhance resident outcomes and facility care standards

Skills & Abilities

  • Strong clinical knowledge of adult and geriatric medicine, with expertise in chronic disease management.
  • Excellent communication and collaboration skills with patients, families, and facility staff.
  • Ability to manage a patient panel efficiently while maintaining a high standard of care.
  • Strong organizational and documentation skills.
  • Commitment to patient-centered, evidence-based practice.

Qualifications

  • Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Georgia.
  • National certification (AGNP, FNP, Adult, or PA-C) with an active, unrestricted DEA license.
  • Minimum 1–2 years of experience as an APP; strong preference for post-acute, skilled nursing (SNF), or geriatric care experience.
  • Mission-minded clinical approach with a genuine passion for senior care and health equity in community-based settings.
  • Strong collaborative communication skills to engage effectively with nursing staff, attending physicians, and resident families

Compensation & Benefits Package

  • Work-Life Balance: Monday–Friday schedule with no nights, no weekends, and zero call obligations.
  • Generous PTO: 4 weeks of Paid Time Off (PTO) plus paid company holidays.
  • Comprehensive Healthcare: Medical, Vision, and Dental insurance plans.
  • Financial Planning: 401(k) plan with company participation after year one.
  • Paid Sick Leave: Dedicated paid sick days for personal well-being.
  • Supportive Culture: High-autonomy practice backed by responsive physician collaboration and clinical operations support
