Job Description
StaffMed Health Partners is recruiting a Post-Acute Care Physician for a leading national physician services organization in a SNF setting in Houston, TX.Perks and Benefits:

- Strong leadership pathway (Facility Medical Director, Regional Medical Director, CMO)*
- Dedicated administrative, billing, credentialing, and scheduling support*
- Clinician wellness program and responsive regional management*
- On-Call: Yes
- Provide attending physician services to a SNF resident panel (typically 75-125 patients)*
- Conduct admission H&Ps within 48-72 hours, monthly recertifications, and significant-change-of-condition visits*
- Manage chronic disease, post-acute recovery, wound care, dementia care, polypharmacy, and end-of-life care*
- Lead and collaborate with NPs/PAs, nursing, therapy (PT/OT/SLP), social work, and consultant pharmacy*
- Participate in interdisciplinary care planning, family conferences, and Medicare Part A clinical reviews*
- Document in PointClickCare or facility EMR per CMS PDPM standards
- BC/BE in Internal Medicine, Family Medicine, or Geriatrics*
- Comfort managing CHF, COPD, diabetes, dementia, palliative needs, and complex polypharmacy*
- Interest or prior experience in long-term care, sub-acute rehab, or skilled nursing strongly preferred*
- Active state license (or eligible) and DEA where applicable*
- Board Certification: yes
