The Post-Acute Community Outreach Specialist educates target groups about services available within the departments represented.  This includes hospitals, physicians or physician groups, care facilities, community organizations, trust divisions, corporations, and community members for the purpose of maintaining continuity of care for clients served or potentially served in post-acute care.  This role develops, coordinates, and implements successful community outreach marketing strategies and events based on an understanding of the market, the service line(s), the health system, its clients and its competitors.WHO WE ARE Franciscan Health is a leading healthcare organization dedicated to providing exceptional patient care and promoting health and wellness in our community.   • Will be supporting rural areas/counties surrounding Tippecanoe County including Benton, Boone, Carroll, Clinton, Fountain, Montgomery, Warren & White Counties.
  • This is part of a health initiative to help with Health Access, Screenings and Chronic Disease Support in our rural communities.
  • Candidate will be expected to train and become a Community Health Worker for this position. Costs associated with this will be covered by Franciscan.
  • Company vehicle will be provided.
  • Coordinates CBISA reporting and troubadour activities to collect information and report on community involvement and program impact on improving health outcomes in local communities.
  • Coordinates the Speakers Bureau to identify and provide speakers on various health and wellness topics to internal and external events.
  • Plans and coordinates booth setup and staffing for health screenings.
  • Develops and implements specific programs and strategies for referrals and referral sources according to the role and department(s) served within post-acute care continuum.
  • Tracks and evaluates effectiveness of strategies.
  • Maintains effective follow-up on prospective referrals and referral sources through personal visits, phone calls, and written communication.
  • Develops long-term relationships with key decision makers in programs and organizations within target market.
  • Maintains contact management database to provide historical record of contact demographic data for each referral account.
  • Provides education and awareness of post-acute services available within Franciscan Health to the community and professional colleagues in a variety of settings for services or service lines supported.
  • Responds and follows up on all complaints and concerns from referral sources and physicians and establishes best practices for the future.

QUALIFICATIONS

  • Bachelor's Degree- Health Promotion, Marketing, Sales or Related- Preferred
  • High School Diploma/GED- Required
  • 2 Years- Health Care Related Sales and Marketing Experience- Required
