What You'll Be Doing
We are looking for a flexible, experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide on-site clinical coverage at a skilled nursing facility in .
- Part-Time Option: Focused strictly on weekend coverage (Saturdays and Sundays from ~9:00 AM–5:00 PM) at a single facility. It is a great fit for a clinician looking for a consistent, predictable schedule without overnight call or weekday responsibilities.
- Full-Time Track Option: For clinicians seeking full-time hours, there is an opportunity to expand the role by covering an additional, nearby building as well.
The role is focused primarily on evaluating and managing new admissions and acute clinical issues that arise, helping residents receive timely treatment in the facility whenever appropriate and avoiding unnecessary emergency department visits or hospital transfers. This is a highly independent role with physician support available as needed.
Key Responsibilities
- Evaluate and treat residents with new or worsening acute medical concerns during coverage hours.
- Respond to clinical issues identified by nursing staff and determine the appropriate evaluation, treatment, and follow-up plan.
- Manage common acute conditions in the skilled nursing setting, including infections, changes in mental status, respiratory symptoms, falls, pain, dehydration, and other changes in condition.
- Determine when residents can safely be treated in the facility and when escalation to the emergency department or hospital is appropriate.
- Review relevant laboratory results, imaging, medications, and recent clinical history to guide treatment decisions.
- Communicate clearly with nursing staff, physicians, residents, and families regarding changes in condition and treatment plans.
- Provide follow-up for residents recently discharged from the hospital or experiencing a significant change in condition when clinically appropriate.
- Maintain timely, accurate, and compliant documentation of all patient encounters.
- Collaborate with the facility and Fortis Care clinical team to ensure appropriate handoff and continuity of care.
Skills & Abilities
- Strong clinical judgment in adult and geriatric medicine.
- Comfortable evaluating and managing acute medical problems independently.
- Ability to quickly assess changes in condition and determine the appropriate level of care.
- Excellent communication and collaboration skills with nursing staff, patients, families, and physicians.
- Strong organizational and documentation skills.
- Comfortable working independently in a skilled nursing facility with physician support available as needed.
- Commitment to high-quality, patient-centered care and reducing avoidable hospitalizations when clinically appropriate.
Qualifications
- Current Nurse Practitioner or Physician Assistant license in the state of Georgia.
- Board certification (AGNP or PA-C preferred).
- Unrestricted DEA registration.
- Minimum 1–2 years of clinical experience preferred.
- Experience in skilled nursing, post-acute care, geriatrics, internal medicine, hospital medicine, or emergency medicine strongly preferred.
- Familiarity with EHR systems and clinical documentation best practices.
Schedule & Options
- Part-Time (Weekend Only): Saturday and Sunday, ~9:00 AM–5:00 PM at one facility. No overnight call. No weekday responsibilities.
- Full-Time Track: Available by expanding coverage to include a second, nearby building.
- Typical daily volume: 12-20 patients, primarily follow-ups and acute issues
