JOB DESCRIPTION: Prevention Specialist PrEP Navigator Job Title: Prevention Specialist Department: Prevention Department Reports To: Medical Director Classification: Exempt Reporting Relationship: Program Manager Supervisory Relationship: None Salary and Compensation: $65K/yr ($31.25/hr) + $747/month benefits = $73,964 total. Job Summary: The Prevention Specialist is an experienced and mature professional who is an essential part of the POCAAN's Prevention Care Team, working under the Prevention Program Manager's supervision. The Prevention Specialist - PrEP Navigator is responsible for providing HIV testing, counseling, and education to people at high risk of acquiring or transmitting HIV and other Sexually Transmitted Infections (STI’s) as well as providing PrEP, enrollment, education, and navigation to HIV negative individuals. The Prevention Specialist position specializes in providing HIV prevention, STI prevention, and safe sex education to gay/bisexual men, transgender, and non-binary persons, as well as high-risk heterosexual females. As a Pre-Exposure Prophylaxis (PrEP) Navigator, you will provide intensive care coordination, support, and services to HIV negative individuals who require assistance in accessing and remaining in PrEP care. This individual will coordinate with the Prevention team to initiate access to PrEP care and the Medical and Pharmacy teams to assist existing PrEP patients. - Weekly check-in meetings with the Program Manager and regular team meetings with the prevention team.
- Weekly maintenance of a work plan which details project goals and outcomes
- HIV 101 Training
- Motivational Interviewing
- PrEP Navigation Best Practices Training
- Participation in other agency training as directed
- Supports the mission, vision, and values of POCAAN
- Complies willingly with all organizational policies and procedures

- Participates in appropriate professional development programs to attain and maintain competency
- Provide testing, education, counseling, and outreach that is specific, sensitive, and culturally relevant to persons of all gender identities and expression/presentation, diverse sexual orientations, communities of color, all ages (adolescents/ adults), and culturally diverse communities throughout all boroughs of King County.
- Provide individual, sex-positive, risk-reduction counseling, including brief PrEP/PEP/STI education/referrals.
- Maintain accurate and up-to-date client testing records for HIV testing and referrals.
- Submit a weekly program and activity progress reports to Prevention Program Manager.
- Occasionally conduct community presentations to increase awareness of HIV prevention and testing in collaboration with the clinical team.
- Conduct outreach to recruit for testing to the priority populations in venues, neighborhoods, or areas where the client may congregate.
- Conduct scientific-based prevention education sessions for individuals at high-risk for HIV infection, including individual sessions, workshops, and social marketing activities for on-going recruitment.
- Help prepare and develop program curriculums, materials, fliers, and all other printed materials required by the program or based on the client’s needs.
- Coordinate with the Wellness Program staff and Prevention Services for health education and materials approval.
- Link newly diagnosed individuals to care services, including case management and doctor appointments, as well as helping with access to health insurance.
- Conduct process and outcome evaluation activities.
- Collect data, perform data entry, and prepare monthly program statistics.
- All other duties as assigned.
- Coordinate and collaborate with other prevention programs and services within Seattle/King County to better serve targeted populations and the community as a whole
- Develop relationships with community leaders and serve as the organization's liaison with various constituents
- Works well under general supervision
- A degree of creativity, initiative, and latitude is required
- Reports daily to a supervisor or manager
- This position will require working some weekends, evenings, and holidays
