You will provide relationship-based primary care, respond to acute needs, and drive measurable improvements in health outcomes—all with full support from Bloom’s interdisciplinary team and advanced data systems.This is the ideal role for a clinician who thrives on independence, enjoys complex primary care, and wants to practice medicine the way it was meant to be: patient-centered, unrushed, and deeply impactful.What You’ll Do
- Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities.
- Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership.
- Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.
- Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.
- Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
- Utilize Bloom’s integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes.
- Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.
- Monday - Friday
- 8-12 patients per day (no unrealistic volume expectations)
- Light call rotation: only 2–3 times per year
- Weekends off and flexible daily workflows
