What You'll Do
Deliver longitudinal primary care
Manage transitional care post-discharge
Reduce hospital readmissions
See patients where they live — private homes, assisted living, skilled nursing (~30%)
Oversee care across a defined regional footprint
Build real patient relationships
Must-Haves
MD or DO
Board Certified/Eligible IM or FM
Active state license (or willing to obtain)
Comfortable with community-based care delivery
Eligible to work in the U.S.
Nice to Have
Experience in home-based, ALF, or SNF settings
Interest in regional leadership growth
