Job description:

The PMHNP Coordinator provides direct patient care and clinical consultation in support of programs. Direct care includes psychiatric assessment, diagnosis, evaluation, and medication management for adults diagnosed with severe mental illness. This role also provides consultation and clinical guidance to staff through collaboration, case review, and treatment recommendations for complex behavioral health conditions.

The provider works as part of a multidisciplinary treatment team, offering direct care and consultative support. This is a 30-hour per week position with a hybrid work arrangement. Work may be performed in-office and remotely via phone or video; however, in-person services are required based on program and participant needs.

Essential Job Functions:

Direct Clinical Care – 75%

  • Conduct psychiatric assessments, diagnosis, and evaluation for adults with severe mental illness
  • Utilize pharmacology within the scope of licensure and Oregon Administrative Rules
  • Provide regular medication services to program participants, ensuring medical oversight of the co-occurring mental health and substance use treatment plans
  • Interpret laboratory and diagnostic test results and provide appropriate treatment modalities
  • Diagnose diseases, ailments, and psychiatric illnesses
  • Provide care in-office and periodically in participant homes, or other locations as needed
  • Complete timely documentation of services rendered into the EHR
  • Provide backup coverage for medical team members as needed due to vacations, illnesses, etc.
  • Ensure compliance with all laws, rules, and regulations governing patient care
  • Meet all ethical and legal standards

Collaboration and Continuity of Care – 25%

  • Collaborate with staff and community care providers to ensure continuity of care
  • Maintain effective, positive working relationships with community agencies
  • Participate in program development and other duties assigned
  • Advise treatment for participants needing more intensive or specialized mental health care until they can be connected to appropriate services
  • Conduct case review consultations, especially for participants not improving under current treatment plans or those with complex medical/psychiatric presentations
  • Participate in collaboration with the Director of Behavioral Health and/or Medical Director, in reviewing medical records and supporting adherence to behavioral health protocols within required timelines

Required Knowledge and Abilities:

  • Ability to synthesize medical and psychiatric data, conduct suicide and risk assessments and formulate DSM diagnosis(es) and evidence based clinical recommendations
  • Ability to communicate appropriately with staff and clients, including individuals with severe mental health symptoms
  • Ability to enter clinical documentation into an electronic health record system.
  • Ability to follow and manage scheduled commitments
  • Proficiency with electronic health records, mobile technology, computers, and standard office equipment
  • Ability to work independently and collaboratively within a team
  • Knowledge of HIPAA confidentiality requirements as they relate to health care and, specifically, behavioral health.
  • Ability to respond effectively and sensitively to diverse populations, including those defined by race, ethnicity, language, age, sex, sexual orientation, economic status, disability, migrant and seasonal worker status, homelessness, people living with HIV/AIDS, and others.
  • Ability to meet Universal Expectations of Employment
  • Ability to perform physical requirements of the position, including lifting up to 20 pounds, standing, sitting, bending, kneeling, crouching, reaching, grasping, tolerating strong odors, speaking, repetitive motion, computer use, and local travel

Minimum Qualifications:

  • Licensed to practice as a PMHNP in the State of Oregon
  • Master's degree in nursing from an accredited college or university and completion of a Nurse Practitioner program specific to the expanded specialty of Psychiatric/Mental Health Nurse Practitioner (PMHNP)
  • Credentialing, training, or experience in addiction medicine or addiction psychiatry to meet Oregon Administrative rules for a LMP competency standards to deliver Integrated Co-Occurring Disorders (ICD) Treatment
  • Experience with or desire to work with individuals experiencing severe mental illness symptoms including psychosis
  • Documentation of current BLS/CPR
  • Documentation of positive clearance from the National Healthcare Integrity & Protection Data Bank and the HHS Office of Inspector General Exclusions Database
  • Eligible for NCQA standard credentialing as LMP
  • Documentation of successful completion of a background and fingerprint check as specified by Oregon law as well as a urinalysis drug screen. The team maintains a drug-free workplace and requires a negative test result for all substances, including marijuana (THC), regardless of state legalization.
  • Valid Oregon Driver’s License in accordance with vehicle use policy and procedures
