JOB SUMMARY:
The Surgical Care Coordinator is responsible for the coordination of patient care and management of surgical patients through the continuum of care. The Surgical Care Coordinator will provide outstanding customer service and ensure staff members provide the same. The incumbent will foster an environment of teamwork and serve as a role model in the department. They will demonstrate actions and attitudes that contribute to the critical success of patient navigation in a community and inpatient setting.
SCHEDULE:
Support Saturday surgeries and vacation coverage.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Assists in the identification of clinic patients eligible for navigation.
- Assesses patient’s ability to navigate through the healthcare system and identifies possible barriers to diagnosis and treatment.
- Provides patient/family with appropriate educational materials
- Assists patient in scheduling appointments for routine, diagnostic and/or follow-up care.
- Assists patients in overcoming financial barriers to obtaining appropriate screening/diagnostic testing and treatment.
- Contacts patients prior to appointments as needed.
- Monitors the receipt of testing and consult results.
- Follows-up with patient on results as directed by the provider.
- Documents significant patient navigation activities into the medical record.
- Ensures coordination of care among treatment providers.
- Develops relationships with personnel and entities providing diagnostic and treatment services to the patient.
- Tracks and trends clinic progress and effectively communicates findings to clinic management and staff to promote screening services, risk stratification and follow-up to surgical patients.
- Participates in Quality Improvement activities as assigned.
- Provides assistance to share the learnings of patient navigation to other preventative and disease state patient navigation initiatives.
- Creates and develops surgical care plans in collaboration with Surgeon.
- Performs other duties as assigned.
EXPERIENCE/QUALIFICATIONS:
- Licensed as a Practical Nurse in the state of Pennsylvania
- A minimum of 3 years of related experience.
- Strong clinical skills, specifically orthopaedics, as well as a well-rounded knowledge of the chronic care management of patients required.
- Must be highly proficient in and knowledgeable of all areas of clinical paperwork and computer use, including electronic medical records.
- Must possess a high level of analytical skills and critical thinking skills necessary to evaluate problems and develop plans and procedures to achieve resolution, assure completion and evaluate effectiveness.
- Independent judgment is necessary to work in accordance with department practices and general directives from leadership.
- Ability to work independently and utilize time management, prioritization and multi-tasking skills.
- Must be proactive, flexible and able to work with different providers and staff to ensure patients’ needs are being met as well as staff satisfaction at an appropriate level.
