Overview

Heritage Home Healthcare is seeking a compassionate and dependable PRN Caregiver for a specific client assignment in Northeast Albuquerque (87111).

This assignment includes assistance with intimate personal care. In accordance with the client’s privacy needs, the client has requested a same-gender caregiver for this specific role.

In this position, you will provide delegated personal care services in a home setting, supporting daily living activities while ensuring comfort, safety, and dignity.

Schedule

Monday–Thursday, 11:30 AM–2:00 PM(Client may prefer a 9:00 AM start time if available)Care is provided by other staff Friday–Sunday.

Key Responsibilities

Assist with personal care tasks including bathing, dressing, grooming, toileting, and eating
Follow prescribed healthcare plans, including assisting with exercises and delegated medication support
Provide mobility assistance (transfers from bed, chair, or wheelchair)
Maintain a clean and safe home environment with light housekeeping
Provide structured support for a client with autism and mental health needs
Assist with organization, scheduling appointments, and errands
Escort or transport client to appointments as needed (reliable transportation required)
Monitor and report changes in client condition to the clinical team

What We’re Looking For

Compassionate, organized, and dependable caregiver
Experience supporting individuals with autism and/or mental health conditions preferred
Strong communication and time-management skills
Ability to work independently and follow care plans
Bilingual (English/Spanish) is a plus
Must have reliable transportation

Qualifications

At least 18 years of age
High school diploma or GED preferred
Previous caregiving experience preferred
Ability to follow delegation instructions accurately
