Overview
The Registered Nurse will be responsible for conducting in-person supervisory visits with EPSDT and VA clients. This role includes managing a caseload of approximately 50–70 clients and ensuring that each client is visited in their home every 60 days. This position is non-clinical in nature and does not involve skilled nursing tasks; instead, it focuses on oversight and client well-being.
The position serves clients located in the southwest quadrant of Albuquerque, specifically those residing south of I-40 and west of I-25. The expected commitment is approximately 50–60 hours per month.
Key Responsibilities:
- Conduct face-to-face supervisory visits with EPSDT and VA clients every 60 days.
- Ensure compliance with all regulatory and organizational standards during visits.
- Assess the overall well-being of clients and provide necessary support and guidance.
- Document visit findings and communicate any concerns or recommendations to the appropriate team members.
- Collaborate with caregivers and other healthcare professionals to ensure coordinated and effective care.
- Maintain accurate and up-to-date client records.
- Provide education and resources to clients and their families as needed.
- Adhere to all company policies and procedures.
Qualifications:
- Current and valid Registered Nurse (RN) license in NM.
- Must reside in Albuquerque or surrounding areas
- Minimum of 1 year of nursing experience, preferably in a home health or community health setting.
- Strong assessment and documentation skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
- Compassionate and patient-centered approach to care.
- Valid driver's license and reliable transportation.
- Must be able to work evenings and have weekend availability for the remainder of the school year.
