To provide MVP members enrolled in the Medicaid Long Term Support Program with the psychosocial support needed to cope with chronic, acute, or disabling conditions. The Social Worker will telephonically assess, identify needs, coordinate, educate, case manage and/or coach members. Services include providing member education and counseling, making necessary referrals to social service programs, and coordinating necessary services for MVP members in the Medicaid LTSS program. Establishes and maintains relationships with community agencies to provide contracted and alternative services when appropriate. Refers members and/or families to community resources to assist in recovery from physical or long-term illness and to provide access to services such as financial assistance, legal aid, housing, or education. Participate in discharge planning and care coordination meetings with multiple providers. Collaborates with other professionals to evaluate patients’ medical or physical condition and to assess member needs. Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan. Follows case management cases to successful completion using appropriate documentation. Performs other duties as assigned by management.
POSITION QUALIFICATIONS
Minimum Education:
Licensed Social Worker is required (LMSW OR LCSW)
Minimum Experience:
Demonstrated clinical experience and previous experience working with the Medicaid population. Previous case management experience required.
Required Skills:
- Excellent oral and written communication skills, including ability to compose correspondence
- Excellent computer skills including strong working knowledge of Word, Windows based applications and the internet
- Detail oriented with strong organizational skills.
- A “team” player able to work well in an energetic, dynamic, and deadline-oriented environment.
- Previous experience required in situations demanding strict adherence to confidentiality.
- Must be able to solve problems while exhibiting clinical judgment and realistic understanding of the current environment.
- Must be able to make independent decisions about appropriateness, resource utilization, and quality of care and an ability to follow regulatory requirements
- Must demonstrate understanding of clinical and psychosocial issues that may alter treatment or care plan and be able to demonstrate good judgment when dealing with emotionally charged situations
- Knowledge of member centric, motivational interviewing strategies, and strength-based models a plus

MVP Health Care is committed to providing competitive employee compensation and benefits packages. The base pay range provided for this role reflects our good faith compensation estimate at the time of posting. MVP adheres to pay transparency nondiscrimination principles.
