Program Manager – Older Adults Home Modification Program (OAHMP)

Position Overview

The Program Manager leads all operational aspects of the Older Adults Home Modification Program, ensuring comprehensive, high-quality, person-centered, and compliant home modification services for low-income seniors. This position is responsible for management of multidisciplinary staff and contractors, direct oversight of project delivery, and rigorous adherence to HUD Section 504, OAHMP program standards, procurement policies, and grant objectives. The Program Manager dedicates the majority of their time (60%) to direct service—including project coordination and hands-on beneficiary support—with an additional 10% allocated to administrative and compliance requirements and 30% to ancillary program development and support.

Key Duties

Program Implementation & Coordination (60% Direct):

  • Supervise daily home modification projects, overseeing intake, needs assessment, scheduling, project execution, and quality assurance.

  • Coordinate and mentor contractors, Home Specialists, and Occupational Therapists to deliver timely, effective, and compliant services.

  • Enforce HUD Section 504 specifications, all maintenance/repair guidelines, and strict 2 CFR 200 procurement compliance.

  • Participate in and review OT-led and CAPS-supported assessments and post-project evaluations.

  • Maintain robust and accurate documentation in Housing Management and Healthy Homes systems (HMS, HHGMS).

  • Provide direct support and clear communication to program beneficiaries throughout each project phase.

Reporting, Compliance & Administration (10% Administrative):

  • Prepare and submit accurate program, compliance, and fiscal reports per HUD, OAHMP, and agency requirements.

  • Monitor grant compliance, procurement standards, and affirmative Section 3 goals.

  • Ensure all contracts, eligibility files, financials, and work orders meet relevant federal and local standards.

  • Lead program audits, quality assurance reviews, and required reporting to funders.

Ancillary Program Duties (30% Other Program Costs):

  • Develop and nurture partnerships (e.g., Built to Last, city housing programs), and coordinate multi-agency initiatives.

  • Facilitate professional development and capacity-building opportunities for all program staff, including HUD and CAPS trainings.

  • Support program innovation, process improvements, and roll-outs of new systems (e.g., Salesforce, GoHighLevel) for project management and tracking.

  • Prepare and disseminate educational resources and Healthy Homes Bundles for participants.

  • Champion continuous improvement, risk management, and outcome evaluation.

Qualifications

  • Extensive experience managing federally funded housing rehabilitation or related programs, especially those supporting aging-in-place, accessibility, or safety.

  • Deep familiarity with HUD regulations (Section 504), 2 CFR 200 procurement, and compliance and reporting standards.

  • Demonstrated expertise in supervising, training, and evaluating multidisciplinary teams, including contractors and Home Specialists.

  • Strong knowledge of OT-led home modification protocols and person-centered models of care.

  • Advanced skills in project tracking, outcome management, and documentation for grant compliance.

  • Certified Aging-in-Place Specialist (CAPS) or equivalent—preferred.

  • Exceptional organizational, analytical, communication, and strategic planning skills.

  • Demonstrated ability to build cross-sector coalitions, prioritize program sustainability, and ensure non-discriminatory, equitable practices.

  • Proficiency in database systems (HMS, HHGMS, Salesforce), Microsoft Office, and workflow automation and client management solutions.

  • Highly valued: Extensive knowledge of GoHighLevel platform for marketing automation, CRM, workflow deployment, and system maintenance, with experience implementing or supporting multi-platform integrations in a program setting.​

  • Required or preferred: Valid General Contractor’s license; in-depth knowledge of regulatory, compliance, and best practices in general contracting, particularly as it applies to residential modifications for senior populations.
