Our Client and Family Support (CFS) Program focuses on empowering families and caregivers of Full Service Partnership (FSP) clients to support loved ones with behavioral health challenges.

    As the Program Manager, you will hold bottom-line responsibility for the CFS Program’s success. You will oversee all program outcomes, reporting, and staff supervision to ensure that we are effectively empowering families, building community resources, and increasing access to services for Sonoma County’s most vulnerable populations. You will also support integration of PHG’s Enhanced Care Management (ECM) and Community Support (CS) services in Sonoma County.

    What This Role Looks Like (Day-to-Day Reality) This is a high-impact leadership role combining program oversight, staff supervision, and community partnership.

    • Hold ultimate accountability for the success of the CFS program and the achievement of Behavioral Health Service Act (BHSA) goals.
    • Supervise, train, and support a team of two field-based Family Support Specialists.
    • Collaborate closely with Sonoma County DHS-BHD, Full Service Partnership (FSP) teams, and PHG’s administrative staff.
    • Monitor data, evaluate key performance indicators (KPIs), and submit regular reporting to County partners.

    What You'll Do

    Program Leadership & Supervision

    • Provide daily supervision and guidance to Family Support Specialists, ensuring they deliver culturally responsive, trauma-informed, and evidence-based services.
    • Prepare, maintain, and ensure staff adherence to the CFS Program Operating Manual, including comprehensive staff training guidelines.
    • Manage the successful implementation of educational workshops and weekly drop-in Resource Clinics.

    Operations & Community Coordination

    • Lead meetings with County personnel and FSP teams to finalize, implement, and maintain streamlined referral and coordination protocols.
    • Act as the primary liaison alongside the Quality Assurance (QA) team to address systemic barriers and coordinate with related County programs.
    • Ensure structured policies, procedures, and strong documentation standards are maintained to ensure program accountability.

    Quality Assurance & Data Reporting

    • Analyze program metrics quarterly with the QA team to address service disparities, develop quality improvement strategies, and maximize community impact.
    • Track service utilization, demographic profiles, and efficiency using the designated Electronic Health Record (EHR) system.
    • Submit detailed Quarterly Reports to the County, outlining initial KPI reviews and action plans for ongoing improvement.
    • Ensure the program is effectively meeting BHSA goals, including reducing homelessness, unemployment, and psychiatric inpatient utilization.

    ECM & CS Integration

    • Support the integration of ECM and CS with Specialty Mental Health Services (SMHS) to create a seamless system of care.
    • Generate ECM and CS referrals through community partnerships, educational workshops, and family resource clinics
    • Collaborate with PHG’s ECM and CS leadership, data and compliance teams to track outcomes and maintain regulatory standards.

    Requirements

    Minimum Qualifications

    • Bachelor’s or Master’s in Healthcare Administration, Psychology, Public Health, Nursing, or related field.
    • 5+ years experience working with individuals with Serious Mental Illness (SMI) and/or complex health and social needs
    • 1+ years leadership experience in behavioral health, social services, or community health program management.
    • Experience developing program procedures, tracking outcomes, and reporting to county or state funders.
    • Thorough understanding of behavioral health systems, local Sonoma County resources, and the Behavioral Health Service Act (BHSA).
    • Excellent communication, organizational, and time-management skills.
    • Proficiency in electronic health records and basic computer systems.
    • Culturally and linguistically competent, with bilingual (Spanish) skills preferred.
    • Strong analytical skills and experience in data-driven program evaluation.
    • Strong leadership and communication skills.
    • Ability to manage multiple priorities in a fast-paced environment.
    • Commitment to person-centered care and health equity.

    Requirements

    • Valid California Driver's License and active auto insurance meeting CA requirements
    • Reliable personal vehicle for daily work use
    • Successful completion of background check (including MVR)|
    • Must be able to travel up to 60-70% within the county to conduct in person visits
    • Must successfully complete a Testlify skills assessment
    • Must have a reliable working laptop for the first 21 days of employment (personal equipment stipend) until company issues laptop is received
    • Must have effective Time Management skills
    • Must have internet speed of - 300+ mbps download and 25+mbps upload
    • Must be proficient in technology, including documentation systems, case management platforms, and communication tools
    • Must be willing to work occasional evenings and weekends as needed.
