Behavioral Health Registered Nurse - Home Care

What You'll Do
As a Behavioral Health Registered Nurse, you will:
- Assess patients' physical, behavioral, psychosocial, and environmental needs.
- Develop, coordinate, implement, and evaluate individualized plans of care.
- Manage an assigned caseload and complete initial and ongoing nursing assessments.
- Monitor patients for changes in condition, safety concerns, and barriers to treatment.
- Communicate important updates to physicians, clinical leadership, caregivers, and other members of the care team.
- Educate patients and caregivers about medications, symptom management, safety, and treatment goals.
- Update plans of care based on patients' changing needs and progress.
- Coordinate services with an interdisciplinary team to support positive patient outcomes.
- Supervise and document care provided by LPNs and behavioral health aides when applicable.
- Maintain accurate and timely clinical documentation.
What You'll Need
- Current, unrestricted RN license in the state of practice
- Associate or bachelor's degree in nursing
- At least one year of clinical nursing experience
- Behavioral health, home health, or hospice experience preferred
- Homecare HomeBase experience preferred
- Dependable vehicle, valid driver's license, and current automobile insurance
- Willingness to travel throughout the assigned territory
- Ability to travel up to 50%
- Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
