Job Details

This role is focused on psychiatric consults for patients in skilled nursing facilities and assisted living communities.

Responsibilities include:

Perform psychiatric evaluations and follow-up visitsManage medication treatment plansCollaborate with facility staff, physicians, nurses, social workers, and familiesProvide consultative behavioral health support to post-acute and long-term care residentsDocument visits and treatment plans in a timely mannerTravel to facilities throughout New London County

Qualifications:

Licensed Psychiatric Nurse Practitioner in ConnecticutPMHNP certification requiredExperience with geriatric, long-term care, SNF, assisted living, or post-acute populations preferredComfortable working independently in the fieldStrong documentation and communication skills
