Physical Therapist (PT) – PRNNellas Autumn Lake | Elkins, WV$55–$60/hour | PRNWe are seeking a Physical Therapist (PT) for a PRN opportunity at Nellas Autumn Lake. Join a collaborative rehabilitation setting where you can provide skilled, resident-focused therapy designed to improve mobility, strength, balance, and everyday function.What You'll DoPerform physical therapy evaluations and develop individualized plans of care

Provide treatment addressing strength, balance, gait, transfers, endurance, and mobility

Establish functional goals and monitor resident progress

Modify treatment plans based on clinical needs and response to therapy

Educate residents and caregivers on exercises and safe mobility techniques

Maintain clear and timely clinical documentation

Coordinate care with PTAs, nursing staff, and other rehabilitation professionals

QualificationsActive West Virginia Physical Therapist license required

Graduate of an accredited Physical Therapy program

Strong clinical, communication, and teamwork skills

Skilled nursing or post-acute rehabilitation experience preferred, but not required

New graduate Physical Therapists are welcome to apply
