Support Patient Progress-Without the Burnout of a Full Caseload

SummitWest Care is a mission-driven, non-profit home health agency serving Mesa County and surrounding communities. We're looking for a Physical Therapist Assistant (PTA) who enjoys working independently, values flexibility, and wants to focus on patient care-without the expectation of managing a regular, productivity-heavy caseload.

This role supports our Physical Therapists by providing treatment visits as needed, helping ensure continuity of care while maintaining balance and autonomy.

What You'll Do

Provide skilled PTA treatment visits in patients' homes under the direction and supervision of a licensed Physical Therapist
Carry out established treatment plans, including therapeutic exercises, gait training, and functional mobility activities
Monitor patient progress and communicate updates to the supervising PT
Complete timely, accurate clinical documentation
Collaborate with the care team while working independently in the field

What We're Looking For

Current Colorado PTA license in good standing
Graduate of an accredited Physical Therapist Assistant program
Valid driver's license, reliable transportation, and ability to travel locally
Comfortable working independently in home settings
Strong communication and documentation skills

Compensation & Benefits

$28–$33/hour, depending on experience
Paid drive time and mileage reimbursement
