PHDHP will function as a part of the quality management team by providing clinical interventions, education for the prevention and control of dental disease, outreach to assist member’s oral health needs, closing dental care gaps and community engagement. The PHDHP will play a vital role in improving oral health outcomes by facilitating access to care and supporting quality initiatives.

Job Description

Essential Functions

  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Serve as an oral health advocate, promoting evidence-based preventive practices and wellness strategies.
  • Perform in person dental services within their scope of expertise at contracted dental offices and dental clinics within the Southeast Medicaid Zone
  • Engage with members via phone or virtual platforms to provide oral health education and counseling.
  • Attend community health events to promote oral health and provide preventive oral health services
  • Maintains accurate records of services performed
  • Participate in quality improvement projects related to oral health outcomes.
  • Assist members with appointment scheduling, transportation coordination, and understanding their dental benefits.
  • Track member interactions and outcomes using company-provided systems and documentation protocols.
  • Collaborate with interdisciplinary teams, including case managers, care coordinators, and provider relations staff.
  • Maintain up-to-date knowledge of state Medicaid and CHIP dental guidelines, regulations, and covered services.
  • Plan and participate in oral health initiatives
  • Participate in quality review meetings to discuss oral health initiative activities

Competencies (Knowledge, Skills, and Abilities Required):

  • Maintain OSHA/CDC guidelines for sterilization, disinfection, and cleaning of instruments and operatories
  • Strong verbal and written communication and counseling skills with the ability to engage diverse populations.
  • Proficient in Microsoft Office Suite and comfortable using electronic health systems and databases.
  • Must be able to travel within the Southeast Medicaid Zone (reliable transportation required).
  • Valid driver’s license and proof of auto insurance.
  • Bilingual (Spanish or other languages commonly spoken in the region) is a plus.
  • Familiarity with managed care organizations and Medicaid managed care regulations.
  • Experience with outreach, case management, or health education.

Minimum Education and Experience Requirements:

Education:

Associates or Bachelors degree in Dental Hygiene from an accredited college or university

AND

Experience:

  • Minimum of 3 years of experience in clinical dental hygiene, public health, or community-based care.
  • Experience working with Medicaid populations is strongly preferred

Minimum Certifications, Registration or License Requirements:

Active Public Health Dental Hygiene Practitioner license in Pennsylvania

Radiology certification

Act 34 Pennsylvania Access to Criminal History (PATCH) clearance

Act 31 Mandated Child Abuse Reporter Training

Current Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) certification
