Job Description
The ClientHBG Action Plan Coordinator (PHPD 2) provides leadership and coordination for Maternal and Child Health Block Grant (ClientHBG) strategic planning, implementation, and performance reporting activities. This position supports the Division's efforts to achieve measurable improvements in maternal and child health outcomes by ensuring effective planning processes, monitoring progress toward established goals, and facilitating collaboration across programs and priority areas. Working closely with program leadership as well as internal and external partners, the ClientHBG Action Plan Coordinator promotes alignment between program activities, federal requirements, and quality improvement principles. The position serves as a central resource for developing action plans, monitoring performance, reporting, and continuous improvement efforts.The ClientHBG Action Plan Coordinator supports statewide maternal and child health initiatives focused on the following priority areas: Increase Access to Contraceptive MethodsImprove Maternal Mental Health and WellbeingImprove the Perinatal Regionalization System in TennesseeDecrease Preventable Illness and Disease Among ChildrenImprove Social Emotional Wellbeing in AdolescentsIncrease Access to Quality Care for Children and Youth with Special Healthcare NeedsImprove Nutrition Among FamiliesKey Responsibilities:Coordinate the development, implementation, monitoring, and reporting of Maternal and Child Health Block Grant (ClientHBG) action plans across all priority areas.Manage project timelines, deliverables, and reporting cycles to ensure compliance with federal requirements. Provide technical assistance, guidance, and writing support to program staff to promote high-quality action plans, reports, and related documentation.Develop and maintain standardized tools, templates, and resources that support consistent planning, monitoring, and reporting practices.Review and analyze action plans and progress reports to identify gaps, risks, barriers, and opportunities for improvement. Supervise and mentor interns, supporting professional development and leveraging intern capacity to advance program goals.Prepare and communicate progress updates, recommendations, and emerging issues to Division leadership and key partners.Minimum Requirements: Bachelor s degree in public health, social work, or a related field; Master s degree preferredMinimum of 2-3 years of experience in maternal and child health, public health program management, or related field; State government experience preferredUnderstanding of maternal and child health issues, disparities, and evidence-based interventionsMust have experience developing, implementing, and managing programs and/or projectsMust pass a background checkThis position requires a high level of written and verbal communication, interpersonal, and leadership skills.
