Public Health Program DirectorRural Health Transformation CHANT ExpansionDivision of Family Health and WellnessTN Residents PreferredThe Tennessee Department of Health s Division (TDH) of Family Health and Wellness (FHW) is hiring a contract Public Health Program Director 1 (PHPD1) to lead the expansion of the CHANT Welcome Home project and acquisition of call center and case management systems, funded by the Centers for Medicaid and Medicare Services Rural Health Transformation (RHT) Program. This position will provide strategic leadership and oversight for the Welcome Home kit project in counties designated as maternity care deserts, and act as a liaison between internal technical teams and software vendors.This role is responsible for overseeing contracts, guiding implementation of funded activities, and supporting community-based partners working to improve maternal and infant outcomes. This position will report to the Deputy Director For Child Health.Key Responsibilities:Provide leadership and oversight for the enhanced care coordination and Welcome Home Kit project across designated rural countiesManage and monitor grant-funded contracts, including reviewing scopes of work, budgets, expenditures, and deliverables to ensure compliance with State and federal requirementsServe as the primary liaison to funded grantees, providing technical assistance, guidance, and ongoing support to ensure the successful implementation of program activitiesMonitor program performance through shared measurement approaches, including review of process and outcome data, quarterly reports, and evaluation activitiesCollaborate with epidemiologists and other program staff to design and implement program evaluation activitiesWork closely with Clinical Application Coordinators in procurement and development of new software platformsSupport development of sustainability plans, including identifying funding opportunities and strategies to maintain program impact beyond the grant periodConduct site visits, facilitate meetings, and represent the program in regional and statewide stakeholder engagementsCollaborate with internal TDH programs and external partners to align efforts and advance maternal, fetal, and infant health prioritiesMinimum Requirements:Graduation from an accredited college or university with a bachelor s degree in public health or a related fieldExperience equivalent to substantial (5 years or more) of full-time, increasingly responsible professional health program workExperience in program or grant management, including contract oversight, budgeting, and reportingDemonstrated ability to lead or support multi-partner initiatives or community collaborativesStrong understanding of public health data and experience applying data to program planning and evaluationExcellent organizational, project management, and problem-solving skillsStrong communication and interpersonal skills, with the ability to engage diverse stakeholders, including community members and individuals with lived experienceExperience providing training or technical assistance is preferredKnowledge of maternal, fetal, and infant health and rural health systems is preferredAbility to travel within assigned regions and statewide as neededAbility to work remotely with periodic in-person meetings in Davidson County and assigned rural locationsIn-Office Requirements:

  1. New hires will be required to come in for the first day of employment for laptop deployment and onboarding in Nashville, and at least monthly, or as requested by hiring manager.
  2. Travel to the Nashville office will not be reimbursed.Typical working hours: Monday-Friday, 8AM to 4: 30PM, 37.5 hours per week
