Full-Time QAPI Resource Registered Nurse (Assisted Living/Memory Care Facilities)

Job Summary

 The QAPI (Quality Assurance and Performance Improvement) Resource Registered Nurse is a leadership and support role responsible for driving clinical excellence, maintaining regulatory compliance, and leading quality assurance programs across multiple assisted living and memory care communities. This position collaborates with facility leadership; Executive Directors, Directors of Health Services, staff nurses and executive leadership to analyze clinical data, prevent adverse trends, and ensure survey readiness.

Essential Duties and Job Responsibilities

Quality Assurance & Performance Improvement (QAPI)

  • Conduct routine and focused audits of resident care plans, medication administration records (MARs), clinical documentation, and infection control practices.
  • Analyze facility data (such as falls, weight loss, pressure ulcers, and infection rates) to identify negative clinical trends and areas of risk.
  • Design, implement, and monitor clinical performance improvement projects and best practices across all assigned locations.

Regulatory Compliance & Survey Readiness

  • Maintain ongoing survey readiness in compliance with state department of health regulations.
  • Lead mock surveys, readiness assessments, and proactive compliance walkthroughs.
  • Assist facility teams during state and local regulatory surveys.
  • Develop, write, and oversee Plans of Correction (POCs) for state citations, tracking corrective actions to ensure sustained compliance

Staff Education & Support

  • Float between assigned facilities to provide hands-on clinical mentoring, leadership support, and operational guidance to Wellness Directors and care staff.
  • Develop and deliver staff training on documentation standards, state regulations, infection prevention, and clinical best practices

Clinical Systems & Collaboration

  • Partner with facility Administrators and regional leadership to review incidents, investigate safety hazards, and implement prompt corrective strategies.
  • Ensure proper utilization and optimization of Electronic Medical Records (EMR/EHR) systems
  • Other duties assigned are appropriate to the position.

Requirements

  • Must possess a nursing degree from an accredited college or university.
  • Must possess a current, unencumbered, active license to practice as an RN in this state. BSN preferred.
  • Nursing experience in assisted living, Alzheimer/dementia, long term care, geriatric rehab, sub-acute and/or critical care clinical setting.
  • Thorough knowledge of CMS and State Regulations regarding patient care in an ALF/IL/MC facility.
  • Experience in electronic medical records documentation.
  • Strong leadership and communication skills.
  • Ability to design and deliver effective training programs.
  • Knowledge of State and Federal regulations and compliance requirements.
  • Excellent organizational and time management skills.
  • Must have and maintain an active CPR/BLS during employment.
  • Maintain confidentiality of all resident care information.
  • A genuine desire to improve the quality of life for seniors.
  • Create and uphold an atmosphere of warmth, patience, enthusiasm, and a calm and cheerful environment.
  • Ability to travel 50 to 80 percent to facilities in region (Oregon, Washington, and Nevada,)
