Current and valid DL or State issued ID required & Utility bill in the Traveler's name with matching address within the last three months of the projected start date. Address must be GREATER than 75 miles from contracted facility
Ability to perform Echo and Vascular. Echo to include bubble study and contrast exams. Vascular to include studies such as segmental pressures, arterial legs, carotid and venous insufficiency exams.
DRIVER'S LICENSE & UTILITY BILL MUST BE INCLUDED WITH SUBMISSION
