Our medical staff includes over 500 providers from Foundation Medical Partners and local practices. Foundation Medical Partners, our multi-specialty group, spans 70+ practices across southern New Hampshire and northern Massachusetts, providing coordinated, patient-centered care to thousands each year.

About the Job:

The Certified Recovery Support Worker (CRSW) plays a vital role in supporting individuals with opioid use disorder and substance use disorder. This position bridges patients, families, community resources, and clinical teams to improve access to care, enhance recovery outcomes, and reduce barriers related to social determinants of health. • Use motivational interviewing to assess readiness for change and support recovery planning.
  • Assist patients in accessing treatment programs, community services, recovery housing, and respite care.
  • Help patients obtain insurance coverage, including Medicaid and presumptive eligibility.
  • Provide individual recovery coaching and facilitate recovery groups.
  • Perform ongoing recovery monitoring and adjust support based on patient progress.
  • Coordinate appointments, transportation, and advocacy across health and social service agencies.
  • Provide education, resources, and support to family members.
  • Conduct community outreach, build partnerships, and introduce Doorway services to individuals in need.
  • Distribute and train patients, families, and partners on Narcan use.
  • Document all interactions accurately and maintain confidentiality per 42 CFR Part 2.
  • Maintain knowledge of referral resources, regulatory standards, and organizational policies.
  • Enter required data into the WITS system.
  • Attend required trainings and maintain active CRSW certification.

Who You Are:

  • A Certified Recovery Support Worker with lived experience or strong understanding of substance use recovery.
  • Skilled in motivational interviewing, crisis response, and trauma‑informed communication.
  • Compassionate, patient-centered, and able to work effectively with individuals from diverse backgrounds.
  • Organized, detail-oriented, and comfortable navigating community resources and healthcare systems.
  • Able to maintain confidentiality, professionalism, and regulatory compliance.
  • A collaborative team member who values advocacy, empathy, and empowerment.

Why You'll Love Us:

  • Health, dental, prescription, and vision coverage for full-time & part-time employees
  • Short-term, long-term disability, life & pet insurance
  • Tuition reimbursement & Nursing Student Loan Paydown Program
  • 403(b) Retirement savings plans
