Location: This position will float between two clinics -
Indian Health Clinic- 703 NE Hancock St., Portland, OR 97212 &
Wellness Center- 12360 E Burnside St., Portland, OR 97233
Schedule: This is an in-person position, Monday through Friday, 8:30am- 5:00pm
Wage Range: $24.15 - $27.30 hourly, Non-Exempt

Position Overview:
The Referral and Patient Care Coordinator provides care coordination to ensure appropriate follow-up of processed referrals. Role encompasses patient navigation to assist with addressing barriers for completion of outside specialty appointments and obtaining medical documentation from referral locations for timely provider review.
Assists with referral processing, resolves pre-certification, insurance, and case-related concerns prior to a patient/client appointment. Gathers pertinent information from insurance carriers, billing staff, provider, and other ancillary staff to make certain the patient/client is not financially obligated for services provided.
Individual will be knowledgeable with all insurance/grant benefits and limitations.
What you will do:
Provides patient navigation regarding referrals that may include, but is not limited to assisting with making appointments for client/patient, arranging transportation, coordination of interpreter services, and modification of referrals based on patient/client preferences
Addresses client/patient inquiries, questions and concerns in all areas including enrollment, claims, benefit interpretation, and referrals/authorizations for healthcare or other resources.
Responsible for receiving, responding to, and directing client/patient, provider, and insurance vendor phone calls. Provides available information upon request and escalates issues as appropriate.
Responsible for following up with referral to determine if completed.
Responsible for ensuring documentation from completed referrals is obtained from outside specialists and present in EHR for referring provider review
Responsible for understanding insurance coverage benefits and limitations.
Documents the client/patient information and grant/insurance billing in electronic health record accurately.
Responsible for tracking all referrals made within medical and dental.
