Ensure Seamless Transitions: Proactively assess patient discharge planning needs and partner with the healthcare team to coordinate smooth, safe, and effective transitions of care.Communicate and Document: Licensure / Certification / Registration:Registered Nurse credentialed from the Oklahoma Board of Nursing obtained prior to hire date or job transfer date required.Diploma from an accredited school/college of nursing OR professional licensure at time of hire required.
