Position Overview:
We are currently seeking a Registered Nurse (RN) Nurse Surveyor to support the New York State Department of Health’s Office of Aging and Long-Term Care by conducting regulatory surveillance, inspections, and complaint investigations of healthcare and adult care facilities. The role focuses on ensuring compliance with applicable New York State and federal laws, regulations, and standards governing adult care facilities, assisted living facilities, nursing homes, and community-based care programs. The RN works as part of an interdisciplinary survey team and applies clinical nursing expertise to regulatory oversight activities.
Responsibilities:
Regulatory & Surveillance Duties:
- Conduct onsite inspections of Adult Care Facilities (ACFs), Assisted Living Facilities, and other regulated programs within a multicounty Central NY region.
- Participate in surveillance and complaint investigation activities related to long-term care facilities.
- Review facility compliance with New York State and federal laws, regulations, and codes.
- Participate in interdisciplinary survey team discussions and decision-making processes.
- Conduct interviews, observations, and document reviews during surveys and investigations.
- Assist in identifying deficiencies related to nursing and medical care standards.
Complaint Investigation & Enforcement:
- Review and triage complaint intakes.
- Conduct onsite and offsite complaint investigations.
- Interview facility staff, residents, and relevant witnesses.
- Summarize investigations in accordance with federal and state principles of documentation.
- Prepare correspondence, complaint closure letters, and investigative summaries.
- Participate in enforcement actions, including testimony at hearings when required.
- Respond to inquiries from providers, complainants, and regulatory partners.
Administrative & Reporting Responsibilities:
- Review and evaluate Plans of Correction submitted by facilities.
- Review facility-generated incident reports, policies, and procedures.
- Assist with applications for new licensed or certified programs.
- Maintain complaint tracking logs and survey case files.
- Utilize electronic surveillance systems and databases, including ASPEN and related tracking systems.
- Participate in meetings, committees, and required training programs.
Medical Oversight Responsibilities:
- Apply professional nursing judgment to regulatory surveillance activities.
- Interpret and apply state and federal healthcare regulations.
- Provide clinical guidance to survey teams regarding nursing standards of care.
- Support quality assurance initiatives within long-term care and adult care programs.

Requirements
Qualifications:
- Education: Associate's Degree in Nursing or higher.
- Required:
- Registered Professional Nurse (RN).
- Current license and registration to practice nursing in New York State.
- Strong written, verbal, and telephone communication skills.
- Proficiency with Microsoft Office applications and electronic documentation systems.
- Preferred:
- Experience in nursing surveillance, regulatory compliance, or utilization review.
- Familiarity with adult care, assisted living, or long-term care programs.
- Experience working with federal and state healthcare databases (e.g., ASPEN).
- Skills:
- Clinical nursing expertise applied to regulatory oversight.
- Strong analytical and investigative skills.
- Excellent documentation and report-writing abilities.
- Ability to manage multiple priorities in a regulatory environment.
- Effective collaboration within interdisciplinary teams.
- Professional communication with providers, stakeholders, and the public.
