Registered Nurse (Rn) - Nursing Home Surveyor

•  Monday – Friday (Flexible hours).
•  Travel required for on-site facility surveys.

•  Active New York State RN license.
•  Nursing degree (BSN preferred).
•  Experience in utilization review, claims, investigations, OR.
•  3 years clinical or administrative experience.
•  SMQT certification (preferred or obtainable within 12 months).
•  Strong analytical, investigative, and communication skills.

•  Conduct nursing home surveys and complaint investigations.
•  Evaluate compliance with federal & state regulations.
•  Document findings and prepare Statements of Deficiencies (SOD).
•  Review Plans of Correction and follow-ups.
•  Enter data into state and federal systems.
•  Participate in hearings, monitoring, and regulatory processes.
•  Travel to assigned facilities as required.
