Registered Nurse – Nursing Home Surveyor / Complaint Investigator – New Rochelle, NY
Location: New Rochelle, NY (travel position with on-site visits)Employment Type: Full-timeRate: $60/hour
Position Overview:
We are seeking a Registered Nurse to conduct nursing home surveys and complaint investigations at facilities in and around New Rochelle, New York. This role involves travel to facilities, investigative work, and documentation to ensure compliance with federal and state regulations and to protect resident health and safety in long-term care settings.

Qualifications:
- NYS Registered Nurse license required.
- A minimum of two (2) years of experience performing utilization review, claims adjudication, medical review, fraud investigation, surveillance, or monitoring activities; OR three (3) years of clinical or administrative experience; OR a Bachelor's degree in nursing and two (2) years of clinical or administrative experience.
- At least one (1) year of survey experience required.
- SMQT certification preferred; must be able to obtain within 12 months of employment.
- Valid driver's license and ability to travel to on-site facilities for review assignments.
Additional Requirements:
- The work schedule is determined by the state; candidates must be flexible with scheduling.
- Must be available to work weekends, holidays, and evening shifts as needed, assigned on a rotating basis.
- This is a travel position.
Key Responsibilities:
- Participate in surveys or complaint investigations, including allegations of abuse and neglect, and perform surveillance activities at nursing homes (long-term care) to determine compliance with federal and state regulations.
- Produce written documentation and draft a Statement of Deficiencies (SOD) within the required time frame, using Principles of Documentation.
- Assess compliance with the Plan of Correction within the required time frame.
- Complete data entry in federal and state reporting databases as required.
- Participate in state monitoring during immediate jeopardy situations.
- Attend meetings as required.
- Testify in administrative hearings as needed.
- Adhere to applicable work schedules, policies, and procedures.
- Travel to on-site locations according to the work schedule.
