Job Description
Apollo Home Healthcare is hiring a full-time or part-time/per diem Registered Nurse (RN) to join our Home Health team, serving patients in Northern California's Bay Area. In this role, you will provide high-quality nursing services to patients in the comfort of their own homes, helping them manage their care and regain independence. You will oversee a supporting Licensed Vocational Nurse and work closely with physicians, therapists, and a supportive clinical team to coordinate care and support patients and their families.

What You'll Do
- Provide in-home nursing visits according to the physician’s plan of care
- Assess patients’ health status, including condition, history, and home environment
- Develop and update individualized care plans based on patient needs
- Administer medications and treatments as prescribed
- Educate patients and caregivers on disease management and home care
- Monitor patient progress and communicate changes to the physician and care team
- Document visits and care in a timely manner
- Coordinate care with therapists, aides, and other team members
- Serve as case manager for assigned patients and support discharge planning
Requirements
- Active California RN license
- CPR/BLS certification
- Strong communication and organizational skills
- Reliable transportation and willingness to travel to patients’ homes
- Comfortable working independently in a home health setting
- Home health experience preferred
- Experience using Kinnser/Wellsky preferred, but not required

Salary Range Disclaimer: The rates listed represent the low and high end of the range for this position. Visit and reimbursement rates vary based on visit type and employment status (1099 or W2). Rates may be adjusted in the future, and employees will be notified of these changes.
