Job Description
Position Overview: The Home Health RN provides skilled nursing care to patients in their homes across multiple counties. This role focuses on delivering high quality, patient centered home health services, supporting patients with ongoing medical needs, and ensuring safe, coordinated care while working independently in the field.
Schedule & Work Details:
- Weekly Hour Guarantee: 40 hours
- Schedule: M-F 8:00 AM - 4:30PM
- On Call and Weekend Requirement: On Call 2-3 times per week and Weekend On Call up to every other weekend
- Coverage Area: Mercer Co, McDowell Co, Summers Co, Monroe Co
- Mileage Reimbursement: Yes, reimbursed between patient visits from first patient to last patient
- Pay Rate: $50-53 per hour
Key Responsibilities:
- Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals
- Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations
- Provides required supervisory visits
- Provides high-quality clinical services within the scope of practice and infection control standards, in accordance with the plan of care, and in coordination with other health care team members
- Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely
- Documents patient visits per policy and payer requirement
- Makes initial and/or comprehensive nursing evaluation visits, ensures patients meet home health eligibility and medical necessity guidelines, determines primary focus of care, develops the plan of care within State guidelines with the physician, and submits accurate documentation
- Communicates relevant information timely and effectively with appropriate agency staff, including patient care issues, visit assignments, schedule changes, orders, OASIS data sets, coding requests, and coordination with other clinicians
- Communicates timely and effectively with physicians, patients, and family members to ensure quality care and service excellence
Required Qualifications:
- Active RN license
- Current CPR certification
- Ability to work independently in a home health environment
- HCHB/PointClick Care experience
- Advanced vascular access management, including PICC line care and Port A Cath access/de access
- Indwelling and suprapubic Foley catheter insertion, maintenance, and troubleshooting
- Venipuncture and lab specimen collection
- Comprehensive wound care management, including complex dressing changes and negative pressure wound therapy (Wound Vacs)
- Tracheostomy care, suctioning, and patient/caregiver instruction
- Ostomy care and education, including appliance selection, skin protection, and troubleshooting
- Strong infection prevention and control practices across all lines, wounds, and devices
