Job Description
Position Overview: Seeking a skilled and compassionate Home Health Registered Nurse (RN) to deliver high-quality clinical care to patients in Simpson County. This role involves comprehensive patient assessments, care planning, and coordination with an interdisciplinary team to ensure optimal patient outcomes. The ideal candidate is experienced in home health, demonstrates strong clinical judgment, and excels in independent patient care.
Schedule & Work Details:• Location: Simpson County• Schedule: Monday through Friday, 8:00 AM to 4:30 PM• Weekly Hours: Full-time schedule• On Call: Rotating call schedule• Weekend Requirement: Included in rotating call schedule• Mileage Reimbursement: Yes, reimbursed between patient visits from first to last visitPay Rate: $51 to $53 per hour
Key Responsibilities:
- Initiate, develop, implement, and revise individualized plans of care in collaboration with physicians and the care team
- Provide skilled nursing care in accordance with the plan of care, scope of practice, and infection control standards
- Perform comprehensive patient assessments including OASIS documentation and medication reconciliation
- Ensure timely, accurate documentation and syncing in accordance with agency and payer requirements
- Supervise and provide direction to home health aides and LPNs, including conducting required supervisory visits
- Coordinate care across disciplines and communicate effectively with physicians, patients, and families
- Conduct initial and ongoing nursing evaluations to confirm patient eligibility and medical necessity for home health services
- Participate in on-call rotation and respond to patient care needs as required
- Contribute to quality improvement initiatives, patient safety efforts, and reduction of unnecessary hospitalizations
- Assist in staff orientation, serve as a preceptor when needed, and participate in survey readiness activities
- Adapt to various home environments and provide care with respect to all patient backgrounds and conditions
Required Qualifications:
- Current and unrestricted Registered Nurse (RN) license in the state of practice
- Minimum of 1 year of home health experience
- Current CPR certification or ability to obtain within 90 days of hire
- Valid driver’s license, current vehicle insurance, and reliable transportation
- Strong communication, documentation, and organizational skills
- Ability to work independently and manage a patient caseload effectively
Preferred Qualifications:
- Experience with OASIS documentation and home health regulatory requirements
- Familiarity with Homecare Homebase (HCHB)
- Strong collaboration skills and commitment to patient-centered care
- Experience supervising support staff and participating in interdisciplinary care teams
