Registered Nurse (Pt) - Homeless Services

The expected salary range for this position is based on several factors including market, funding, candidates experience level, skills, training, and related knowledge. Our ranges are developed and published as good faith estimates for the position.

Starting Salary: $48,000 + Office Benefit Package (On-call expenses): $90/Month subject to change

Hours: Tuesday and Friday (9 am - 5 pm), Thursday (half day), 20 hours per week

RADIAS Health is hiring a Registered Nurse to provide professional, quality program services to the persons served by the PATH Team. Promote the physical and mental health of clients on the PATH team by monitoring, educating, and coaching clients as needed. Work with other members of the treatment team to assist clients to obtain and maintain an optimum quality of life consistent with the needs and strengths of the homeless population. Provide nursing leadership for all aspects of client care.

Duties and Responsibility

• Provide PATH services (outreach, screening, case-management, re-engagement) to PATH eligible/enrolled clients.

• Within the nursing scope of practice, provide medical and psychiatric care to individuals experiencing unsheltered homelessness in Ramsey County, on their own terms and in their own lived environment, to build relationships and deliver medically necessary services for individuals to stay healthier to avoid unnecessary Emergency Department visits and hospitalizations.

• Take the lead role and participate in providing medication administration, education and medical services.

• Provide wound care in camps and shelters, including assessment, dressing changes, and escalation of infectious or non-healing wounds.

• Perform basic foot care and skin assessments.

• Assess and triage for health complications and manage within scope of practice or refer to appropriate medical providers or emergency department.

• Evaluate and chart psychotropic medication effectiveness, complications and side effects, and arrange for required lab work according to protocol.

• Participate in regular case consult meetings.

• Manage the system of distribution of medication to clients and collaborate with PATH team members to deliver medications or arrange pick up from preferred pharmacies.

• Administer injectable medications to clients using best practice. Maintain a tracking system of LAI antipsychotic and extended-release buprenorphine due dates. Generating advanced reminders to prevent lapses.

• Provide community outreach and engage with people living in places not meant for human habitation as well as in shelter settings and community drop-in locations.

• Engaged in coordination outreach efforts with RADIAS Health outreach staff and other community outreach providers.

• Complete nursing tasks based on standing order set appropriate for camp / shelter environment. (standing order set from Street Medicine Institute Guidelines)

• Distribute naloxone and provide overdose recognition and response education. Provide harm reduction education and supplies per program protocol.

• Maintain a record of controlled medications and samples and manage the destruction of outstanding medications.

• Perform medication reconciliation at each encounter and after hospital or crisis residence discharges.

• Coordinate prior authorizations and pharmacy communications for psychiatric and addiction medications.

• Coordinate, schedule, and complete medical assessments of clients physical health, make appropriate referrals to community physicians, and coordinate psychiatric treatment with medical treatment providers to ensure continuity of care.

• Work with external providers to encourage consistent and appropriate ongoing care of the client.

• Assess the need for hospitalization, crisis residence, or detox services.

• File reports with Vulnerable Adult, the Ombudsman’s Office, Child Protection, and other mandated/required reports when appropriate.

Requirements

Required: Current license as a Registered Nurse in the State of Minnesota

Preferred: Bachelor's degree and experience in inpatient, outpatient, or community psychiatric setting
