Job Description
REGISTERED NURSE
Job Summary
We are seeking a dedicated and compassionate Registered Nurse (RN) to provide skilled nursing services and health oversight for individuals in the West Virginia Medicaid Waiver program (I/DD).
Key Responsibilities
- Assessment & Care Planning: Conduct initial and ongoing nursing assessments (quarterly/annual reviews) to determine medical eligibility and individual health needs. Develop, evaluate, and revise individualized care plans in collaboration with the interdisciplinary team (IDT), physicians, families, and direct care staff.
- Medical Oversight & Case Management Support: Monitor the health status of individuals, document changes, and communicate with physicians and other healthcare professionals. Ensure all physician orders for medications and treatments are current, properly documented, and implemented.
- Training & Education: Provide health-related training and guidance to unlicensed personnel (e.g., LPNs, AMAPs, Direct Support Professionals) on an individual's specific medical needs and interventions.
- Documentation & Compliance: Maintain accurate and up-to-date health records, medication administration records (MARs), and other required documentation in compliance with WV Bureau for Medical Services and waiver program regulations.
- On-Call Support: Participate in an on-call rotation to provide nursing support
Qualifications & Requirements
- Education: Graduate of an accredited school of nursing.
- License: Active, unencumbered Registered Nurse (RN) license to practice in the state of West Virginia or a compact state license.
- Experience:
- Minimum of two (2) years of nursing experience (preferred).
- Experience working with individuals with intellectual/developmental disabilities or the aging population (preferred).
- Experience in home healthcare or community-based settings is highly desirable.
- Skills: Strong medical assessment, organizational, and communication (written and oral) skills. Proficient in computer skills for documentation and electronic systems.
- Other:
- Must have a valid driver's license, reliable transportation, and valid auto insurance
- Ability to perform tasks required to support individuals in the program and maintain a safe environment.
- Successful completion of required background checks
