Job Summary (List Format): - Active Alaska RN License Required: Must possess a current, unrestricted RN license in Alaska at time of application submission. - Flu Vaccination Required: Must be vaccinated against influenza. - Start Date: Assignment begins no sooner than 3 weeks after fingerprint cards are received in Juneau. - Patient Care Management: Actively manages SEARHC patients by collaborating with medical providers and interdisciplinary teams to set individualized healthcare goals. - Critical Thinking & Problem Solving: Utilizes critical thinking, problem-solving, and anticipatory skills to address patient and provider needs promptly and efficiently. - Referral Coordination: Works with providers and clinical staff to facilitate and coordinate patient referrals to Surgery/Specialty Clinics; reviews records, tracks progress, and provides updates. - Primary Clinical Contact: Serves as the main clinical contact for patient care and care coordination; identifies and addresses delays or obstacles to care. - Staff Nursing Duties: Performs nursing duties such as patient assessment, documentation, education, and administration of medications and therapies within the clinic. - Maintain Competencies & Safety: Maintains annual competencies including safety and infection control; ensures a clean and safe work environment and adheres to policies and procedures. - Teamwork & Communication: Collaborates effectively with team members within and across departments; participates in department and clinic improvement initiatives. - Additional Duties: Performs other assigned duties, including special projects as needed to support comprehensive health services. - Education Required: Graduate of an accredited nursing program (Bachelor's preferred); high school diploma or GED required. - Experience Required: Minimum of 1 year of case management experience. - Certifications Required: Basic Life Support (BLS) certification. - Core Competencies: Proficiency in the nursing process, customer service, safety and infection control; strong assessment, communication, and data collection skills; ability to multitask and work independently; team player with strong problem-solving and teaching abilities.
