Job Description:

Private Duty Registered Nurse

You became a nurse to lead care with purpose.

On An Average Day You Will:

(includes, but not limited to)

  • Complete comprehensive admission assessments
  • Determine appropriateness for home care services
  • Develop individualized plans of care
  • Coordinate with referral sources for smooth transitions home
  • Communicate with physicians, caregivers, and interdisciplinary team members
  • Supervise RNs, LPNs, and Home Health Aides
  • Review documentation to ensure alignment with the plan of care
  • Reassess and update plans of care based on patient progress
  • Coordinate equipment, pharmacy, and community resources
  • Ensure quality and continuity of care

What You Bring to the Team:

  • Graduate of an accredited RN program
  • Active RN license in the state of practice
  • Two or more years of clinical or home health experience preferred
  • Strong communication and organization skills
  • Ability to lift, push, pull, or support up to 50 pounds
  • Commitment to patient centered care
  • Ability to work within an interdisciplinary team
