What You'll Do

As a Resident Care Manager, you'll play a key leadership role in ensuring residents receive exceptional care while supporting and guiding the clinical team.

Responsibilities include:

• Coordinating and overseeing daily nursing care in collaboration with physicians and the interdisciplinary team

• Leading and supporting licensed nurses and CNAs through coaching, guidance, and daily oversight

• Assisting with admissions, transfers, and discharges while ensuring accurate documentation

• Completing and managing MDS assessments and related clinical documentation

• Conducting care conferences and collaborating with residents and families to create individualized care plans

• Monitoring resident conditions and ensuring care plans are updated and followed appropriately

• Promoting a positive and supportive environment for residents and staff

What We're Looking For

• Current Registered Nurse (RN) or (LPN) license required

• Strong leadership and team-building skills

• Experience in skilled nursing or long-term care preferred

• Knowledge of care planning, MDS, and regulatory compliance

• Passion for providing high-quality resident-centered care
