Job Description

Job Description:

The Complex Discharge Planner plays a vital role in managing patient flow from Concord Medical Center (CMC) and Walnut Creek Medical Center (WCMC) to post-acute care settings, including Skilled Nursing Facilities (SNFs), Residential Care Facilities for the Elderly (RCFEs), board-and-care homes, and other lower levels of care. This position supports system-wide efforts to reduce hospital length of stay by ensuring timely transitions, leasing post-acute beds when needed, and helping patients access benefits and resources that support safe discharge and long-term success. The RN also plays a key role in ensuring that patients receive the right care, at the right time and place. The Complex Discharge Plannerhas a relentless focus on excess day reduction and care transitions when patients do not require general acute care. This role is utilized for community members who require a higher level of case management than typical post-acute services, helping to manage demand across the care continuum.

Education

  • Bachelor of Science in Nursing (BSN)- Required

Experience

  • At least 3 years of clinical experience in acute care, with 1 or more years in case management, discharge planning, or post-acute care coordination (Skilled nursing, Residential Care or Payor, etc.)
  • Prior experience with leased bed programs, patient throughput management, letters of agreement, or transitional care coordination- Preferred
  • Experience working with diverse and underserved patient populations- Preferred

Certifications

  • RN Registered Nursing- California Board of Nursing- Required
  • BLS Basic Life Support- American Heart Association- Required
  • Case Management Certification (CCM, ACM, or similar)- Preferred

Knowledge, Skills, and Abilities

  • In-depth understanding of post-acute care services, hospital discharge processes, Medi-Cal and other benefit programs.
  • Excellent communication, critical thinking, financial analysis, time management and continuous improvement skills. Ability to build strong relationships with physicians, interdisciplinary teams, patients, families, and community partners.
  • Proficiency in Epic Electronic Health Record required.

Work Shift:

08.0 - 08:00 - 16:30 No Waive (United States of America)

Pay Range:

$96.12 - $129.76

Hourly

Offer amounts are based on demonstrated/relevant experience and/or licensure.

Pay will be adjusted to the local market if hired outside of the Bay Area.

Note: Positions at JMH which are exempt (not eligible for overtime) under the level of Manager are listed as hourly for compensation purposes on this posting. The work shift will contain the word 'exempt' on it.
