• Valid RN licensure in the practicing state
• Minimum of 2 years of relevant nursing experience
• Current Basic Life Support (BLS) certification from the American Heart Association
• Proficiency with computer systems, healthcare software, and web-based applications
• Strong verbal and written communication skills
• Excellent organizational and time-management abilities
• Effective conflict resolution and sound judgment

Nice to Have Skills:

• Certification in Case Management (CCM) or Healthcare Management (CPHM)
• Experience with care management policies and evidence-based criteria

Preferred Education and Experience:

• Associate’s Degree or Bachelor’s Degree in Nursing (BSN highly preferred)
• Proven track record in patient care management and care coordination

Other Requirements:

• Ability to adapt communication style to diverse patient populations
• Knowledge of federal, state, and local healthcare laws and standards
• Willingness to work flexible hours, including potential shift variations
• Ability to lift and move objects weighing up to 50 pounds as required by physical demands
• Occupational exposure risk to communicable diseases and hazardous materials, with appropriate safety protocols in place
