We are seeking an experienced RN - Case Management to lead and optimize patient journeys through the healthcare system. This pivotal role involves managing care plans, ensuring seamless transitions, and delivering exceptional outcomes that impact lives daily.   • Valid RN license in the state of practice
  • Minimum of 2 years of relevant nursing experience
  • Proficiency in computer applications, electronic health records, and web-based tools
  • Strong verbal and written communication skills
  • Excellent organizational and time management abilities
  • Sound judgment and decision-making skills in complex situations
  • Ability to collaborate effectively within multidisciplinary teams
Nice to Have Skills:
  • Certification in Case Management (CCM) or Healthcare Management (CPHM)
  • Experience with care coordination in diverse healthcare settings
  • Bilingual abilities or additional language skills
Preferred Education and Experience:
  • Associate’s Degree or Bachelor’s Degree in Nursing (BSN preferred)
  • At least 2 years of experience in case management or similar roles
Other Requirements:
  • Basic Life Support (BLS) certification from the American Heart Association
  • Knowledge of current federal, state, and local laws, accreditation standards, and regulatory requirements
  • Ability to adapt care plans to diverse patient populations and settings
  • Willingness to work in environments with potential exposure to communicable diseases and hazardous materials
  • This position is scheduled to start on 03/25/2026 and is a 24-hour shift (12:00 AM – 12:00 AM).
Compensation for this role is highly competitive, with an hourly pay rate of $57.71.
