RN Hospice

0.8 FTE 64 Hours a Pay Period

Days, Hours between 8am-4:30pm

Weekends: Every 5th-7th

Holidays: Every 5th-7th

Position Overview:

A registered nurse (RN) is a health care professional skilled in all phases of the nursing process. Delivery of care is directed toward education, promotion, and restoration of health; prevention of disease; and care of the sick and disabled. He/she is adaptable to the needs of the individual and family during health and illness and has the ability to prioritize. The RN assesses, plans, implements and evaluates daily patient care, and is responsible for the direction of staff members assigned to him/her. The RN is responsible and accountable for the delivery of individualized nursing care to patients receiving Hospice interventions. This position may include other duties as assigned. Required to collect, interpret, record and communicate pertinent data that is relevant to the patient's care through client interview, observation, review of records and consultation with other members of the health care team.

Essential Duties & Responsibilities:

  • Conforms to the Nurse Practice Act.
  • Remains up-to-date on Condition of Participation for Hospice, and current Hospice nursing practice, medications, and legal issues related to patient-based services.
  • Follows recommended infection control policies during patient contact.
  • Possesses the ability to assess and interpret all patient findings including all body systems and in-home needs.
  • The RN utilizes data collected to develop an individualized plan of nursing care that reflects the use of current knowledge, available resources, patients' rights, and communication with the individual, family, and other health care professionals. This plan of care is developed in collaboration with other health care team members and the Hospice Interdisciplinary Team.
  • Collaborates with the physician/provider to develop a medical plan of treatment that meets the health care needs of the patient and family. Revises the plan of treatment with the physician/provider as the patient's health care needs change.
  • Exercises informed judgment and uses individual competence and qualifications as criteria in seeking consultation, accepting responsibilities, and delegating nursing activities to others.
  • Maintains competence in all technical skills needed to deliver client care (i.e. venipuncture, infusion therapy, aseptic technique, etc.). Reports malfunctioning equipment promptly and removes from the patient care area to maintain a safe environment.
  • Guides and teaches patients/families toward "self-help" independence in recognition and solution of physical, emotional and environmental health problems and accepting responsibility for care.
  • Exhibits ability to make decisions independently, appropriately and considering the well- being of all involved. Acts as a resource person for all members of the health care team.
  • Identifies, labels and dispenses medication accurately and provides patient education
  • Maintains communication with the Hospice office team, RN case Manager and Interdisciplinary Team to assure patient needs are met. Communication includes but is not limited to: scheduling, patient status changes and community needs, etc.
  • Performs nursing activities in an organized and timely manner, maintaining productivity standards.
  • Continually works to gain better understanding of workflow of the department and uses knowledge base to analyze department processes and makes changes to improve efficiencies.
  • Ongoing education and in-depth understanding of Continuous System Improvements (CSI) and supports standard work.
  • Participates in and supports continuous improvement event initiatives.
  • RN is knowledgeable of CMS Condition of Participation for Hospice. Completes documentation and patient assessments according to CMS Rules and Regulations.

Supervisory Responsibilities:

Supervises home health aide caregivers to assure adequate patient care is provided in accordance with the medical and nursing plan of care. Orients the home health aide to the plan of care for each patient and supervision continues on every two-week basis.

Skills and Experience:

Required:

  • 2- or 4-year nursing degree. Graduate of an accredited school of nursing. CPR Certification. RN License- MN. RN License- WI. Basic computer skills. Word. Excel. MatrixCare or another database. PowerPoint. Outlook

Preferred:

  • BSN preferred. Hospice certification desirable.

Required Competencies:

  • This position is a direct care provider and requires the ability to give adequate care to patients/residents in the following age groups:

Child (2-12)

Adolescent (13-17)

Adult (18-65) Geriatric (65 and over)
